From cwtv.com:
A young couple is murdered in their home shortly after buying an antique painting of a family portrait circa 1910. Upon reviewing the painting's provenance, Sam and Dean learn that everyone who has ever bought the painting has been murdered and race to discover how the portrait is causing the deaths before it can claim its next victim.
The episode was written by David Ehrman and directed by Phil Sgriccia.

YIKES! I was watching this a few nights ago (in the dark - can we say STUPID??). The scene where Sara and Sam are in the house freaked me out! Especially the part where Sara is trying to help Sam get out from under the furniture and she looks up and sees the girl right there...I jumped and yelped! Poor Lucy jumped up, ready to defend me. LOLOLOLOL

Aww. Lucy, the defender I liked Sarah a lot; I'm still hoping that Sam will run into her again eventually. Maybe at the very end of the show? On another note ... can you believe german tv chose NOT to show this episode? They want to end this season one week earlier to show something else, so they left out this ep. So german viewers (who haven't seen the original eps yet) got to see "Dead Man's Blood" and hear Dean referring to Sarah and have absolutely no idea who he is talking about. And even worse ... they want to show this ep somewhere during Season 2. How's that for continuity? I wonder how they will explain Sam's hair LOL

Oh, yeah, that girl was creepy.
I liked Sarah as well and would like to see her back.

I brought Sarah back!!!
After Madison, I think Sam is done with women... I don't think he'll let himself go back to sarah or attach to another...

I don't think that he'll be done with women forever. Eventually he's going to find the right one ... who he won't have to shoot because she's a werewolve and who won't burn on the ceiling because a demon has plans with Sam. You gotta have faith

Okay--the other day I read that Jensen made an error in this episode that wasn't caught and therefore stayed in. Supposedly, in one of the scenes, instead of saying "Sam," he says "Jared." Today, I found the scene. To me, it sounds lke "Jared," but it could be because he's talking kind of soft. It's at about the 26 minute mark, when they are discussing the razor in the painting. If you have the time to watch it, would you tell me what you think?
What else I found fun about watching this--there are quite a few bloopers on the gag reel from this episode. It's fun to see how they came out correctly compared to all the laughing on the gag reel!

DAMN! it's SO obvious once you hear it.
When they get to the lady's house... Just as Dean notes the difference...
DAMN!
Excellent!

So I'm not crazy ;D? I never noticed when I watched it (I guess because he says it so softly). I wonder if they realise it now?
