From cwtv.com:
While hunting a Djinn, a genie, Dean is attacked and transported to a world where his mother (guest star Samantha Smith) is alive, Sam is in law school and engaged to Jessica (guest star Adrianne Palicki), and Dean lives a very normal life with his girlfriend (guest star Michelle Borth).
However, after he starts seeing a strange girl (guest star Melanie Neige Scrofano) and learns all the people he has saved in the past have died, Dean must decide whether he wants to stay in this new safe life where everyone he loves is alive or if he should return to the hunt.
Raelle Tucker wrote the episode directed by creator/executive producer Eric Kripke.

Here's two questions for you:
1. Why does everyone keep telling Dean "get some rest" all the time? Was that explained somewhere by Kirpke or someone else? Because I'm sure it means something.
2. Dean's estrangement from Sam - do you think it's his subconscious "wish", or a simple side-effect of all the other events of that different life?

1. I never even noticed that! Huh, I'll have to rewatch.
2. I think it's Dean's subconscious saying that, if things had been different, he and Sam simply wouldn't be as close as they are. They are so different (on the outside anyways), so I think he has this deep fear that, if they didn't have their twisted family history, that he and Sam would have become estranged down the line.
Hmm, maybe that's one of the reasons they broke up contact while Sam was in Stanford? Maybe Dean was afraid Sam wouldn't want to have anything to do with him anymore, now that he was leading a "better" life.

I totally agree with what Sil said about the enstrangement, and I think that played a part in Dean questioning his reality.
As for the "get some rest," I think that was how Dean was feeling said aloud. He'd had a rough go of it (his dad dying, the whole thing with saving/killing Sam, being chased by the FBI, then Sam getting possessed), and I think it was his mind saying it had had enough, that he just wanted to be able to rest and not have to worry about anything. I think Dean carries the weight of the world on his shoulders, and he was getting tired of it all.
That's a good question..if any of us ever meet Kripke, we should ask it!

I had the same impression as you did Mick, I just wanted to know if it's just a gut feeling or if someone ever explained it. Good to know I'm not the only one who thought that.
