Dean pressures Sam into visiting Cicero, Indiana, so he can visit an old flame named Lisa (guest star Cindy Sampson), he had a tryst nine years ago. However, Dean is stunned when he meets not only Lisa again, but also her 8-year old son Ben (guest star Nicholas Elias), who bears a striking resemblance to him.
However, Ben soon goes missing and Sam and Dean discover that a Changeling, a demonic parasite, is snatching kids from the small town and assuming their form. Meanwhile, Ruby (Katie Cassidy) makes contact with Sam and reveals some startling information about his mom.
Phil Sgriccia directed the episode written by Sera Gamble.

I found this over at TWOP. Someone copied/pasted it from another forum. I hope it's okay to copy here. We did one thing on the first episode I did this year, called The Kids Are Alright. There was a scene at the end where Dean's saying goodbye to Lisa, played by Cindy Simpson, and I asked Cindy, "Do you think you would kiss him?" [The kiss wasn't scripted.] And she's like, "Well, of course I want to kiss him, but should I?" And I said, "I'm not going to tell him." So we filmed it without Jensen knowing that she was gonna move up and kiss him on the lips. And the shot that's in the show is his reaction. He stayed right in character, but it was a really heartfelt moment because bascially he's saying, "I gotta go," and she's saying, "Well, thanks for saving my son." His reaction just makes your heart go awww. So we're hoping the audience responds to things like that.

That was my first thought upon reading this, too!
I think the fact that Jensen is able to remain in character says something about his ability to become the character he is playing.
